Some background music would really sell the PTSD portion as well as more sound effects. Adding a downside to failing to catch a fish might be nice as well.
EDIT: also music would be nice. Especially if it's really chill then turns super tense during the PTSD.
Sound adds so much to this experience. The heartbeat and the shots in the background and the panic of trying to reel in the fish while the time is running out. At first I felt that waiting for the fish to bite was a little too boring, but after I caught a few I realized that the wait is actually your chance to relax while you're listening to the birds before the next PTSD episode. This game is deep, it's an experience, it's the stuff.
I like the idea you had for a fishing mechanic
The mood was set really well, the first time I caught a fish I really felt the stress. However, I wish there was more to the game. Like something you could do with the fish you've caught, might have been me missing something.
One thing I have to mention is that this game feels awfully similar to [**THIS**](https://ldjam.com/events/ludum-dare/48/fishing-trouble) submission from the last Ludum Dare
Also, the art is really cute but maybe fish takes a bit long to bite.

I am especially impressed by the way the audio and visual transitions between each of the game states, It's very smooth and convincing.
Sound design overall is flawless, I honestly can't find anything bad about it. Visually, some extra animations on the fish would have been nice. Still, I found it very cool how if you fail the fish swims away to the side, that's a nice touch!
The only thing I would say could improve the game would be a bit more variety in the gameplay. Maybe having different minigames to be chosen at random in the PTSD phase would have helped here, so you're always on the edge about them and the unpredictability. I think paired with the sound design it would really kick up the mood up a few notches.
But overall a very solid and polished entry to the jam!
The calmness of the fish before hooking and the unstable of the fish after hooking create a strong emotional contrast.
