I ran into the same bullets-on-ground bug, but I also once saw bullets moving with an enemy that were not attached to that enemy by any path of bullets (there was blank space between the enemy and those bullets). \n\nSeems like the best strategy is to isolate an enemy and then pump them full of bullets to get all the combo points.
