Collateral by Jonathan Whiting
I typically make non-violent games, so getting a theme like this threw me initially. I did find a concept that I liked in the end, and I'm really quite happy with how it all came together, I hope you like it.
Edit: I've put up a playthrough video for anyone that might be having problems getting back out. It's spoilerific though: http://www.youtube.com/watch?v=meKITP12Dsc

i'm stuck at the same bit as Sparky! *keeps trying*
It's something of an inspiration for future LDs for me as well, to nail down the basic art and mechanics completely on day one to allow for level design. It demonstrates how that extra time pays off.
Didn't have a problem with the difficulty level.
This is one of the best LD entries I've ever played.
SUCCESS!!!
Would be good to have some in-game documentation about pressing Z to go through the cutscene dialogue and the [spoiler] mechanic that people seem to be having trouble with (I found it by pure luck). Also, some better sound effects and voice acting for the cutscenes would be awesome.
At one point a high-pitched sound kept playing over and over again, so I had to turn off the sound effects.
I think your game would do well if you put it on a flash portal.
You could probably make higher resolution graphics and work on the sound more, but I think the gameplay and level design are pretty solid.
Then I watched your video... Which shows an entirely different game...
